Office Policies
At Fort Malden Dentistry, we want your first visit with us to be as easy and hassle-free for you as possible. We've made our office policies available here, so that patients can review them ahead of time, to be better prepared for their first appointments.
Change of Appointments
Appointments are scheduled specifically for you. When you schedule your appointment, you will receive an appointment card with all the details of your upcoming visit.
As a courtesy reminder, our receptionist will contact you by phone, or by email, a few days prior to your upcoming appointment to confirm the time and date.
We would appreciate a minimum of 2 full business days’ notice to change or cancel each appointment.
Payment Options
We accept the following methods of payment:

- MasterCard
- Debit
- Cash
Medical Updates
When you schedule your first appointment with us, we will ask you to complete a New Patient Registration Form prior to your first dental visit. It is very important that you keep us informed about any issues regarding your health.
We also ask that you provide us with an up-to-date medication list at the first appointment. It is important that you continue to keep us informed about any medication changes at subsequent visits. We will ask you to update your information during each dental visit.
Insurance Benefits
As a courtesy to our patients, we will assist you in filing your insurance forms related to the dental care you received at our office. Many plans do not cover the entire cost of the specific treatment that you require.
It is your responsibility to know the details of your benefit plan. We do not have access to this information. Payment for the portion of cost not covered by your benefit plan is expected on the day of treatment unless other financial arrangements have been made with us in advance.
Privacy Policy
The Personal Health Information Protection Act, 2004 (PHIPA) came into effect on November 1, 2004, and governs the collection, use, and disclosure of Personal Health Information within Ontario’s health care system, and the Personal Information Protection and Electronic Documents Act, which came into effect in 2001, governs Personal Information other than Personal Health Information.
